NOTE: If you are renewing a level 2 or 3 expired license (expired for more than a year), you will be required to submit a New (Initial) Online Application, meet current licensure requirements and submit a $150 processing fee

All processing fees submitted are NON-REFUNDABLE, and the application and fee are valid for one (1) year from the original submission date (i.e., if you applied on 7/23/22, your application and fee are valid until 7/23/23).
